NOTE - if you do not have a scanned image you have the option to mail us your documentation.

Please upload a scan of your UPC barcode label and a copy of receipt / invoice.
You may attach up to 2 scanned file / images.

After you have selected your file, if you see a blinking yellow image to the left of your file for an extended period of time.
Click 'Cancel', then 'Remove' and re-try.

It is recommended to keep copies of all materials submitted for your records.
